WHAT TO EXPECT
New Patient Appointment
Your first appointment may take up to one hour. We ask that you bring in your current insurance card, a valid id and any discs containing previous radiology studies. We are particularly interested in the CT scan and MRI’s that pertain to the reason for your visit. Results of evaluations performed by other doctors are also helpful. We ask that you arrive 30 minutes prior to your scheduled appointment time to allow adequate time to fill out your New Patient Paperwork and to allow time for our staff to input your information into our system for your provider to review. Download New Patient Paperwork. You can start completing these forms at home. Filling out the forms ahead of time is not only more efficient but it gives you time to think about your symptoms before your visit. This will help you to best explain your problem to your provider.

Follow Up Appointment
We encourage you to bring in your current insurance card and a valid photo ID. We ask that you arrive 15 minutes prior to your scheduled appointment to allow adequate time to fill out your Follow-up paperwork and to allow for staff to input your information into our system for your provider's review. Download Follow Up Paperwork. Please be aware that you will be asked to fill out paperwork for every follow-up appointment. We understand that this can feel repetitive, but we want to ensure that your provider has the most accurate and up-to-date information in order to give you the best possible care.

EMG / NCS Appointment
Your EMG / NCS appointment may take up to 45 minutes. We ask that you bring in your current insurance card and valid photo ID. You should arrive 15 minutes prior to your scheduled appointment time to allow adequate time to fill out the EMG paperwork.
We ask that you refrain from wearing any oils, lotions, creams or perfumes on the area to be tested. The results of the EMG testing will be sent to your referring provider within 24 business hours so that they may review the results with you.

PRESCRIPTION REFILLS
To refill your prescription, please contact your pharmacy to send an electronic refill request.
